Pre-treatment chest CT ( a ) demonstrated ground-glass opacity, reticulation, traction bronchiectasis, and honeycombing predominantly located in bilateral lower lobes, in accordance with subpleural fibrotic ILA. The tumor was located in ILAs and presented the lobulation sign ( a , b ). The patient underwent fine needle aspiration biopsy, with a pathological diagnosis of adenocarcinoma without EGFR mutation.
